Care Management

Care Managers visit clients at home to provide clinical assessments that determine which services accommodate each client's unique needs.

In-home professional care services include:

• Nursing (acute and chronic care)
• Physiotherapy
• Occupational Therapy
• Social Work
• Speech and language pathology
• Nutritional and dietary
• School Health Support

Home and Community Health Care staff work in partnership with other health care professionals and organizations in the community and region to help our clients achieve and maintain good health. Some of our service partners include the Victorian Order of Nurses, Partners in Rehab, Bayshore Home Health, and Comcare Health Services.

Community and Personal Support Services

Supportive care that assists people with the activities and challenges of daily life.

Personal Support Services

Dilico's personal support staff assist clients with their daily living activities, personal care and hygiene and home management activities. By following clearly defined care plans, personal support workers provide assistance with routine activities of living and home management services.

Care plans may incorporate the following services:

Dilico provides supervised group acivities and workshops for Anishinabek Elders and adults. Group sessions support well-being through health education, exercise and social interaction.
